Our integrated model combines psychiatric nurse practitioners and licensed clinical social workers to provide a full spectrum of mental health services at no cost to the facility. We handle scheduling, credentialing, and billing so our clinicians can focus on what they do best: patient care.
Position Overview
We are seeking a Licensed Clinical Social Worker (LCSW) to provide psychotherapy and psychosocial support to residents in skilled nursing facilities. The LCSW will conduct assessments, deliver individual and group therapy, collaborate with the interdisciplinary team (IDT) and ensure regulatory compliance.
Key Responsibilities
Conduct initial diagnostic assessments and develop individualized treatment plans.
Provide psychotherapy services tailored to resident needs.
Deliver individual, group, and family counseling focused on depression, anxiety, grief, adjustment, and behavioral health issues.
Document sessions in compliance with Medicare and facility requirements.
Participate in care plan meetings and interdisciplinary team discussions.
Support residents and families in coping with illness, placement, and end-of-life issues.
Collaborate with Strive psychiatric nurse practitioners and facility staff to ensure coordinated care.
Educate facility staff on recognizing psychosocial needs and appropriate referral processes.
Qualifications
Master’s in Social Work (MSW) with active LCSW license in the state of Wisconsin.
Enrolled or eligible for enrollment as a Medicare Part B provider.
Experience working with geriatric populations or in long-term care preferred.
Strong interpersonal, documentation, and communication skills.
Ability to work independently within a collaborative, team-based environment.
