Job Responsibilities: Case Manager Salary: $20-30/Hour Company: Aetna Location: USA Educational Requirements: Bachelor's Degree Full Job Description: As a case management coordinator, you will use critical thinking in collaborative cases and inform the case management process. To ensure appropriate health care outcomes for members, you will coordinate care, support and training members using care management tools and resources. You will conduct comprehensive audits using management tools and information/data analysis. In this role, you will also have: - Plan and implement assigned care plan activities and monitor care plan progress. - Conduct multidisciplinary research to achieve the best results. - Identify and expand the quality of care through established channels. - Use negotiation skills to obtain the appropriate options and services needed to meet the member's health care needs and/or benefits. - Use effective/motivational interviewing skills to ensure maximum team engagement and promote lifestyle/behavioral changes to achieve optimal health. - Provide guidance, information and support to enable the member to make healthy and personal health and/or lifestyle choices. - Help the member to actively and effectively work with their provider in making health care decisions. - Use case management and quality control procedures based on legal and accreditation standards and company policies and procedures. The rate of pay represents the regular hourly or monthly rate of pay for all positions and job classes covered by this position. The actual salary will depend on various factors, including experience, education, geography and other important factors. This position is eligible for CVS Health cash, commissions, or part-time incentive programs in addition to the compensation limits listed above.
